* Page 13’s not in the members service record.
* Commanding Officer’s endorsement not signed or present in package.
(This endorsement is by the COMMANDING OFFICER ONLY).
* Missing VE score and TIR/TIS in page 4 or Electronic Training Jacket (ETJ).
* VE score does not meet requirements (50 or greater) for assignment.
* Ensure that you have a waiver if your VE score does not meet the minimum.
* Tattoos visible while in PT gear but no pictures in the package.
(Tattoos are not an automatic disqualification as appropriate tattoos can be waived)
* TIR/TIS not met for assignment.
* Waiver requests should be submitted with packages for PO2s and Junior PO1s.
