LDO/CWO Required Paperwork/Items to Bring

Below is the required paperwork needed for each student reporting to Officer Training Command Newport.  You will receive all paperwork turned in to us back at the end of the course.

Mustang University Required Paperwork

• Permanent Change of Station (PCS) Orders and any Modifications
• Copies of prescriptions from civilian physicians if taking prescription medication. At Minimum 1 weeks’ worth of medication should be hand carried with you.
• All medical/dental records and immunization records.  Military and Civilian.
• Oath of Office
• All DD-214's (CWO’s Only.  LDO’s, keep your enlisted service record, no DD214’s)
• Commissioning/Sea Duty Screening Physical
• Oversea’s Screening (if applicable)
• Pre-commissioning PFA documentation (bring PRIMS print out).
• Also the POC for your last command PFA Coordinator (Name/Rank and contact info)
• Direct Deposit Information (Your banks routing number and your account number).
• Direct deposit is not optional. Bring your bank account number and financial institution's routing number (RTN) for your direct deposit. You will be offered the opportunity to open an account with Navy Federal Credit Union but this is not mandatory.
• Page Two (Record of Emergency Data).
  -It would be acceptable if you re-signed the two copies of the current page 2 on the last page under your previous signature. Your new signature means that there are no changes to the current page 2, and that it is up-to-date. After your new signature, put today's date.  Without a page 2, we cannot pay you Basic Allowance for Housing (BAH) Type 2.
• Updated SGLI forms
• BAH Verification Paperwork
• Copy of marriage/separation/divorce papers (for changes since detachment)
• Transfer FITREP/EVAL as an officer from the date of your commission to your transfer date, unless you commission on your transfer date.
• Last PSD Point of Contact info (POC Name/Phone/Fax/and Address)

** Note, blank forms (e.g. Pg-2, SGLI, Direct Deposit sign-up, etc.) will be available to fill-out during Admin check-in session in class.

Note: Make sure you get your commissioned ID card and your dependents ID’s are updated before departing so DEERS is updated.  After commissioning, contact DEERS to ensure continuity of entitlements.

Other Items:

**Special Dietary and Religious needs must be expressed on the first day of training.  OTCN will do everything possible to accommodate all special and religious needs without compromising training. 

• Money:  Meals will be paid for by each student.  Money is collected on a weekly basis by the company mess treasurer.  Meals cost approximately $10.80/day (Breakfast $2.30/Lunch $4.25/Dinner $4.25) for a weekly cost of $75.60.  You should bring enough cash to handle the first two weeks of meals ($151.20).  You will have access to an ATM machine.

• Service Khaki uniform with ribbons and all required components
• Combination cover
• Khaki garrison cap
• Navy Black (Eisenhower) jacket (with liner recommended)
• Navy Black leather shoes
• Navy issue raincoat (with liner recommended October – May)
• Navy issue watch cap and gloves (October – May)
• Navy issue PT gear shorts
• Navy issue yellow PT short sleeve shirt(s)
• Navy issue yellow PT long sleeve shirt(s)
• Navy issue PT Blue sweat pants
• Navy issue PT Blue hooded sweat shirt
• Running shoes with white socks
• Conservative swimwear (swim trunks or PT shorts for males; one-piece black or blue swim suit for females)
• Bath towel, wash cloth, toiletries, etc.
• Shower shoes
• Mesh Laundry Bag
• Combination or key lock (1)

Recommended Items

• Civilian clothes for liberty
• Casual dress pants and casual dress shirt with collar for class dinner, Officers’ club, etc.
• Civilian winter jacket, gloves, etc  (October – May)
• Writing tablet

Authorized Items

• Laptop
• Cell phone
• Alarm clock or small clock radio
• Bath robe
• Brown uniform shoes
